What kind of insurance do you accept?
Our providers participate with a variety of different insurance plans. As always when you are interested in seeing a new provider it is advised that you contact your insurance carrier to make sure the provider is in network with your particular plan. Furthermore, it's important to make sure that your plan is for our location at 2800 S. State St.
It's a great opportunity for you to ask questions and get familiar with your benefits. We will do our best to assist you in understanding your benefits but we do encourage you to take a proactive role in all aspects of your health care including insurance. How can I get refills on my prescriptions?
Refilling your scripts is easy! The best way is to use Patient Fusion our online portal or you can Email or give us a call several days before you run out of your medication. We ask for 24 - 48 hours to handle your request. Tell us your name, medication, dosage and the name and location of the pharmacy you would like your prescription sent.

I need to see a doctor immediately, what are my options?
If this is a true emergency, you need to call 9-1-1 or go to the ER immediately
If you need to see the doctor, please give us a call. If you are a current patient, we will make every attempt to fit you into the schedule or have the doctor call and talk to you. It is often difficult to schedule new patients for immediate appointments. Please remember that we are a small, independent practice and there may be times that our schedule is completely booked. If this happens and you need to see a doctor immediately, we suggest that you visit an Urgent Care for treatment.

How do I log into my Patient Fusion Account?
You should have been sent an invitation to join the portal by the office at your first appointment. Please note that you cannot sign up for the portal using the appointment reminder note about the portal. Once you receive the invitation, it's easy to set up your account. You can either call us to get the access code or you can request that the code be sent to you using your cell phone. The invitation is only valid for a month so if you do not use it, you need to contact us to obtain a new invitation. To log into Patient Fusion go to www.Patientfusion.com
